Send the following five documents:
Immediately report any injury to your work location supervisor. The supervisor should direct the employee to the Workers' Compensation contact. ALL work locations should have a backup.
A Notice of Injury must be completed even if the injury is considered minor. "Save" it in the Workers' Compensation Portal and do "not" click "send to TPA", unless the employee seeks medical attention.
An injured worker must receive treatment from an authorized Workers' Compensation physician. Please see the Workers' Compensation contact at your work location for assistance.
NO, you must go to an authorized Workers’ Compensation doctor.
The work location Work Comp Contact or the backup gives the injured employee an authorization for initial medical treatment. All subsequent care must be authorized by USIS at 407.352.0374.
Go to the Employee Self Service on the intranet, choose OCPS Apps, then choose Workers' Compensation App.

LOD days are absences the Division of Workers' Compensation and OCPS allow for WC pay due to a NO WORK status from an authorized treating physician. Ten (10) LOD days are allowed per fiscal year/injury, whichever comes first. LOD days are reported to payroll when the work location receives written documentation from the employee, the payroll clerk will then report the absences directly into SAP. Once the ten (10) LOD days are used, the payroll clerk will enter (with proper documentation) WC without pay into SAP. The employee can call his/her adjuster at USIS for an explanation of benefits.
The injured employee is released from their normal work day to attend any authorized WC medical appointments. Documentation for these appointments must be provided to the work location supervisor and to the WC office. The employee is allowed appropriate travel time to and from these appointments. Approximate time for physical therapy is one (1) hour plus travel time; doctor appointments vary, but should not exceed two (2) hours. The employee is required to work the remainder of the scheduled work day. The WC office requests that the employee schedule these appointments when it is least disruptive to their work day. The Workers' Compensation injured employee is not docked time for authorized appointments, nor is this time charged to LOD.
The whole day is charged to the employee's own sick or personal leave.
Without written documentation all absences are charged to the employee's own sick or personal leave.
The work location notifies the Light Duty office when an injured employee returns to work with restrictions. While on Light Duty it is the employee’s responsibility to document daily attendance on a Sign In/Out Sheet. The employee is required to fax the sheet to the regular work location supervisor and the Light Duty office every Wednesday. The employee is required to report any absence to the regular work location (following normal procedure), the Light Duty office, and the Light Duty worksite. The employee remains on the payroll at the regular job location. The regular school/work location is responsible for reporting all absences, leaves, etc., while the employee is on Light Duty. A certificate of absence is required.
