Hello Nursing Staff!

We will be piloting a new entry system for the eligible Medicaid services that you provide on a daily basis. In order to implement this the Medication Log and the Documentation of Health Services Forms have been updated. These forms are attached below. Your participation should not result in any additional work, all that is require is that you complete these forms for your ESE students who have an IEP, Medication &/or Health Service Plan of Care or Specialized Plan of Care (ie, Diabetes, Seizures, etc.). To participate, please enter the information requested on the form and send completed form to judy.lopez@ocps.net.
