Welcome to the OCPS Workers’ Compensation Web site. All employees of OCPS are protected against the cost of injury arising out of or in the scope and course of their employment in accordance with Florida Workers’ Compensation law. Any employee who is injured on the job is required to immediately report the injury to the appropriate supervisor and complete the required forms and must be treated with an authorized treating Workers' Compensation doctor.
We are committed to providing prompt and timely, appropriate medical care to the injured employee. We provide a network of highly qualified, authorized treating physicians to provide medical treatment.
We provide a Light Duty program for benefited employees. This program is designed to keep the injured employee with work restrictions in an active status until released by the authorized treating physician to return to full duty work.
We are dedicated to providing a safe working environment for all employees.
In case of a serious injury call 911 and report the nature of the injury so the appropriate medical transportation can be dispatched. Contact the Workers’ Compensation (WC) department at 407.317.3904 or 2003904, if an injury requires emergency medical transportation to a hospital.
The Florida Workers’ Compensation law has a penalty of up to $1,000 for late reports. The work location is responsible for payment of incurred penalties. The work location has seven (7) days from the date of knowledge of the injury to file a Notice of injury (NOI) if the employee has either lost time or seeks medical treatment..
Any person who knowingly and with intent to injure, defraud, or deceive any employer or employee, insurance company, or self-insured program, files a statement of claim containing any false or misleading information commits insurance fraud, punishable as provided in Florida Statute 817.234.
Rewards of up to $25,000 may be paid to persons providing information to the Department of Insurance leading to the arrest and conviction of persons commit insurance fraud. Persons may report suspected fraud to the department by calling 1.800.378.0445. A person is not subject to civil liability for furnishing such information, if such person acts without malice, fraud, or bad faith.

Please print the Employee Handbook Basic Safety Rules and place in your employee handbook. These basic safety guidelines were developed by the Workers' Compensation office and USIS Safety Consultant. Please provide all your employees the Basic Safety Rules and include the importance of safety at your weekly meetings.

Instructions for processing new claims in the OCPS Portal WC APP.
Basic instructions for processing and tracking all Workers' Compensation Claims. Workers' Compensation contact should have all required forms printed ahead of time in a notebook/folder.
This form must be maintained by the work location contact. Required information shall be documented on the Tracking Log. The employee is required to read and sign this form.
This form should ONLY be completed if the WC APP in the portal is not working. Then and only then the employee and the work location contact MUST complete this form together. Please report to the work location contact for assistance.
This form must be signed by the Workers' Comp. contact. Then the form is taken to an authorized Workers' Compensation treating physician.
This brochure provides basic information for the injured employee. Each injured employee must receive this brochure.
This letter must be taken to the pharmacy when the treating physician prescribes medications.
This form must be completed and signed by the injured employee and their administrator, and sent to the Workers' Compensation office within five working days of the reported injury.
The employee is required to sign this form granting authorization for USIS to request medical records.
This form must be completed annually by the Workers' Compensation Contact or anytime the contact changes at the work location.
