Templates and Forms Guidelines

Instructions: Open the required form and select "Save As" to place a copy of the form on the computer.

  • RIGHT CLICK on the document then click on SAVE TARGET AS and save the document to your computer.
  • Do not CHECK OUT a document, instead close the document and then use the instructions above to download a copy to your computer.

Do not save information in online forms!

Process Management Template Descriptions
 

  • Business Plan (BP) - A business plan is a document describing a company's core business activities and how it plans to achieve its goals. (Business Plan Instructions)
  • Customer Instructions (CI) - Explains "HOW" each step identified on the CEM will be completed by the customer. This form includes pre-requisites, and what to expect after the form is received.
  • Metric Definition (MD) - A worksheet used to define the measurement and the calculation of key performance indicators to ensure validity and reliability of the data.
  • Problem Statement (PS) - A simple method of identifying specific issues in a program or service as part of the Environmental Scan process used in planning.
  • Process Inventory (PI) - A worksheet used to define process steps, process owner and collaborators or stakeholders, documents linked to the process, and quality assessment metrics.
  • Process Workflow (Word or Visio) (WF) - A high-level graphic depiction of when or how a process begins, a hand-off to other offices or departments, and how the process ends when completed satisfactorily.
  • Standard Operating Procedure (SOP) - A low level step-by-step provided to employees that explains both "What" steps are required to complete the process satisfactorily and "How" the steps are completed satisfactorily. SOP include pre-requisite information, employee steps, and customer steps. This form also includes communication methods for change management.
  • SWOT Analysis (SWOT) - A framework for identifying and analyzing the internal and external factors that can have an impact on the viability of a project, product, place or person.

    • ECS - Environmental Compliance and Sustainability
    • FNS - Food and Nutrition Services
    • PS - Procurement Services
    • SEM - Safety and Emergency Management
    • TRANS - Transportation Services

    Names for documents will be designed to include key words that are searchable:

    For example:

    “Name of Procedure”_“Department Acronym”_“Office ID”_“Type of Document”

    • Names will not include revision dates or any indication of revision. The revision number and/or revision date will be included within the document footer.  The date in an SOP will indicate the last date of revision.
    • Proprietary documents will include “Proprietary and Confidential” in the document footer and will not be imported into CCDOCS.

  • Some guidance documents are included to assist in making documents accessible to everyone.  Please check all documents to ensure they are ADA Compliant.  Most documents have an Accessibility Tool available.  Contact the OCPS Legal department for questions regarding ADA Compliance.
